TestMyHormones.com is focused on building

patient relationships that are completely based

on privacy and confidence.  All personal patient

information including name, address, e-mail

address, credit card number, sales purchases,

telephone number & lab results are completely private and

confidential.  All information provided to TestMyHormones.com

is kept in a secure, locked, and password protected location. 

Third Parties

Information, including lab results are never disseminated to any

third party without proper legal release from the patient.  In

addition, personal information is never sold,

rented, or leased to any third party. 

Order Policy

All orders are placed on a secure server which

is set up to primarily protect all patient

information.  Shipping details are shredded

on-site following completion of all on-line orders.

Return Policy

TestMyHormones.com will accept certain test kits

for return as long as they are unopened and in a

condition that will allow them to be re-sold.  All

approved returns will be subject to a 15% restocking

fee that will be deducted from the return amount.  Once

a test kit is opened the customer can no longer return

that kit.  To return a kit the customer must first call

TestMyHormones.com to receive a RIC (Return

Identification code).  This code must then be written

on the outside of the box or envelope in which the kit

is mailed in.  Returned items will not be accepted or

credited without an RIC code.   Returns are only

accepted up to 30 days after the date of receipt

of the kit by the customer.  Due to the nature of

the items purchased TestMyHormones.com reserves

the right to not accept returns items based on the

information listed above.

NOTE: California Residents - Business and Professions Code 1246.5 Requires That Any Test that has not been approved by the Federal Food and Drug Administration for sale to the public without a prescription as an Over-The-Counter Test kit must be ordered by a Licensed physician acting on a patient's behalf.  All orders for test kits within the state of California

NOTE: New York State Residents New York State health law prohibits the testing of certain specimens collected in or mailed from New York, and prohibits the transmission of data from our laboratory to NY physicians or residents. Therefore, direct receipt of lab results for NY residents is not possible.

International Residents & Orders:  Extra shipping charges may be added for international orders.  Patients will be contacted by e-mail or telephone to approve charges.
